All students are required by law to submit proof of immunizations.
This form should be completed by a physician and returned to the following address:

Failure to submit this form will result in a hold being placed on your University records, including your transcript following completion of your semester of study at Fordham. Students are strongly advised to bring a copy of this form with them for their records.
Students for whom it is not possible or practical to have this form completed by a physician in their home country may use the services of the Fordham University Health Center to get required immunizations for a fee. The Health Center is located in McMahon Hall, Room 203. You should call 212.636.7160 for an appointment.
Health Insurance and Other Fees
International students who attend Fordham on student visas are required to have adequate health insurance. Students will be billed for such insurance and enrolled in Fordham's student health insurance plan unless they provide proof of other adequte coverage by the stated deadlines.
As an international student at Fordham, you will be responsible for the following fees (reflects fees from the 2012-2013 academic year; subject to change in 2013-2014):
International Student Health Insurance (unless waived): $362 (fall), $736 (spring; also covers summer)
International Students Service Fee: $39/semester
Technology Fee: $100
General Fee: $213
